Forms for a New Tenancy

Form # Form Name Description
45 Application Screening Guidelines Form is used to let the tenant know what your criteria is, so they can decide if they would qualify before turning in the application.
1 Application to Rent Form enable applicants to tell you about themselves so you can decide whether or not you'll rent to them.
2A  Rental Agreements Month to Month and Fixed Term Agreement is the basic contract between you and your tenant.
2B Fixed Term Rental Agreement This form is used for a tenancy for a fixed period of time.
11, 51, 54, 9 Smoke & CO2 Alarm Acceptance, Lead-based Paint Disclosure & Mold Agreement, Check in Check Out Conditions Report These are addendums to your rental agreement and should be signed with every tenant.
21 Deposit Refund If you take a Security Deposit, this tells your tenants what your expectations are when they move out. This can be giving at the beginning or end of tenancy.
3 Pet Agreement Form to get the description of the pet and the rules for a pet
20 Application Verification This form enables you to screen potential tenants 
24Cable/Satellite/Security/System Installation AgreementThis form is used when your tenant says they want to install Cable, a Satellite Dish, or Security System that will include wiring on the outside of the building.
27 Smoke Free Addendum No smoking policy
33 RulesThis addendum is essential to make clear what the tenant should be doing or not doing to protect your property and others
53 Reasonable Accommodation Request If your tenant asks you to have an assistance animal or make changes to your property or your policy due to a disability.
46 Assistance Animal Agreement. Remember, an assistance animal is not a pet and must be treated differently

23  Actual Damages When a tenant breaks a rule or causes damage and you incur costs as a consequence, you are entitled to be compensated for your costs. 
26 Personal Property Notice Warning about leaving personal property unsecured.
28 One-Time Late Payment Use if your tenant is going to pay the rent late.
29 One-Time Partial Payment Use if your tenant is breaking the rent in 2 or more payments.
31 Guest Registration Keeps track of guests
41 Annual Recycling Notice Informs tenants about recycling options.
57 Temporary Occupant Sign if your tenant wants a long-term guest.

Warnings and Termination Notices

Form # Form Name Form Description
4 72 Hr Notice to Pay or Vacate for Non-Payment of Rent This is the basic form for terminating the tenancy of a tenant who hasn't paid rent.
5 Notice of Termination - No cause Notice that tenancy is being terminated. 30 day or 60 day if tenancy has been over 1 year. Cannot be used if tenant is on a lease.
38 Notice of Termination with Cause A 30 day notice
6 Pet Violation Notice Use for unauthorized pets
7 Repeat Violation Termination Notice Use when violations are repeated.
8 Notice for Harm or Substantial Damage Use when the behavior is especially bad.
34-35  Parking Violation, Warning Notice Notice of a parking violation and warning for prohibited behavior 
 44144-Hour Notice of Non-payment of Rent This is an alternative form for terminating a tenancy because the tenant hasn't paid rent.

After the Tenancy Ends

12 Deposit Accounting The "whys" & "how much" when you are not returning any or all of the deposit. Must be mailed within 31 days after termination.
16 Confirmation of 30-Day Notice
This form establishes that you received the notice and spells out some details that are often the subject of disagreement later
21 Deposit Refund If you take a Security Deposit, this tells your tenants what your expectations are when they move out. This can be giving at the beginning or end of tenancy.
25 Abandoned Property Notification, if the tenant left belongings behind.
49, 50 Accounting Letter Promissory/Installment Note Forms that might help you collect what you are owed.
